The game opens shortly after the asylum, once abandoned, has been reopened by a private corporation. The game's asylum has a long, confused history that includes being used as a front for CIA-backed MKUltra experiments, Morin said. Those experiments, which are still shrouded in secrecy and that some think continue to this day, dealt with drug use, torture and some form of mind control. The backstory is drawn from the real MKUltra experiments conducted by the CIA in the 50s, 60s and 70s in the U.S. The game takes place in a fictional asylum set in the real forests of Mount Massive, a Colorado " fourteener" that is the second highest peak in the Rocky Mountains, about 20 miles east of Aspen. "So he's got to find a way out and at the same time investigate and see what's behind those experiments, who's behind those experiments, what's been going on with the patients." "He figures it's probably a good idea to just leave, but it's already too late and he's trapped inside," Morin said.
The reporter goes there to investigate, but when he arrives he realizes that all hell has broken loose already. Outlast tells the story of a reporter who gets a tip from an anonymous informant that something fishy is going on at a mountain asylum.

In another vision, you find her hanging in the hallway after an apparent suicide at Blake’s old Catholic school. In one version of the events, you follow Jessica’s screams to find her with what appears to be a broken neck as she lays injured on the staircase. We’re still unsure of how to achieve these variations but we’re sure it’ll give us insight to what really happened to Jessica after you left her with Father Loutermilch at school that day. The other version has a harsh rope mark around her neck with a pale blue hue to her skin. One version of Jessica looks normal with no indication that something terrible has happened to her. While the results are nearly identical, something you do during the game will trigger a change in Jessica’s appearance when you find her in the school cafeteria at the end of the game.
